Hazel Duck Pritt
  About Hazel's name.  First name may actually be Duck and her middle name is Hazel, like in her marriage license.  It is unknown whether Duck was a nickname or her actual name.  Her name has always been Duck....Period.  Someone on Find A Grave has "assigned" her the middle name of Belle. This is not accurate. The middle name of Belle does not appear in the historical records anywhere.  Her grave marker has Hazel D Forinash.

Multiple family trees on ancestry.com have copied this information with no documents to back-it-up.  How do I know this?  All of these family trees have Hazel's birth date as Nov 1979.  No day.  If any of these family trees were created by the actual direct family members, they would know the exact date of death.

  About husband Adam's first name.  The only place in the historical records that has Addison as Adam's first name is the 1900 Census.  Period.  Someone on Find A Grave decided to go with Addison despite his grave marker that says Adam M Forinash and all of the other historical records that also just say Adam.  And 100 people on Ancestry.com decided to copy Addison.
